Job Summary
Provide and document skilled nursing care in patients' homes under the supervision of a Registered Nurse, following individualized care plans and physician orders. Responsibilities include assessing patient condition, coordinating and delivering nursing care, documenting actions and outcomes, updating the nursing care plan, ensuring infection control per OSHA standards, educating patients and families on disease processes and self-care, and maintaining professional competencies. Requires graduate of an accredited nursing program, active LPN/LVN license in the practicing state, CPR certification, ability to travel, and organizational skills. Flexible scheduling available (full-time, part-time, or PRN); duties occur in patients' homes with travel to multiple assignments; state-specific licensure requirements apply.
Required Qualifications
- Graduate of an accredited school of nursing
- Active, unencumbered LPN/LVN license in the state of practice
- Current CPR certification
- TB skin test (within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Must have reliable transportation
- Compact licenses must be transferred to your state of residence within 90 days
- State-specific requirements: South Carolina – pediatrics experience; California – 1 year experience; Louisiana – 1 year experience
