Job Summary
Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under RN supervision per care plan and physician orders in patients' homes. Responsibilities include patient observation and reporting changes, updating nursing care plans, ensuring infection control per OSHA, educating patients and families, maintaining accurate documentation, and adhering to policies and regulatory requirements. Requires active LPN/LVN license in the practicing state, CPR certification, TB test, and transportation; preferences include pediatrics experience and recent clinical nursing experience. Shifts include full-time or part-time, with days, nights, and weekend availability; travel to patient homes as needed.
Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- TB skin test
- Six months prior hands-on nursing experience preferred but not required
- Must have reliable transportation
