Job Summary
Licensed Practical Nurse/LVN (LPN/LVN) in private-duty home healthcare. Responsibilities include delivering and coordinating skilled nursing care under RN supervision per patient care plans and physician orders, documenting nursing actions and assessments, updating and implementing the nursing care plan, recognizing changes in patient needs and taking appropriate actions, ensuring infection control per OSHA standards, educating patients and families on disease processes and self-care, and maintaining nursing competencies through ongoing education and skill evaluations. Roles support days, nights, and weekend shifts with flexible scheduling and home-based patient visits; requires travel to patient homes within service areas (Round Rock, Georgetown, Hutto and surrounding areas).
Required Qualifications
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification (hands-on component)
- TB skin test current within last 12 months
- Six months hands-on nursing experience preferred but not required
- Reliable transportation
- Compliance with state-specific requirements (e.g., pediatrics experience in SC; California/Louisiana experience requirements)
