Job Summary
Licensed Practical Nurse (LPN/LVN) providing skilled home healthcare under RN supervision in patients' homes across South Austin, Kyle, Buda, San Marcos, and New Braunfels. Delivers and documents nursing care per care plan, observes and assesses patient conditions, communicates changes to supervisor/physician, updates nursing care plans, and ensures infection control per OSHA standards. Educates patients and families on disease processes, self-care techniques, and prevention strategies; maintains compliance with Nurse Practice Act and organizational policies. Requires active LPN/LVN license in the state of practice, CPR certification, ability to travel to patient homes, and strong documentation and clinical assessment skills. Six months of recent nursing experience in a clinical care setting and home health experience preferred; continuing education and state-specific requirements apply.
Required Qualifications
- Graduate of an accredited school of nursing
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification
- TB skin test (within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
