Job Summary
Licensed Practical/Nurse (LPN/LVN) provides skilled nursing care in patients' homes under RN supervision per care plans. Responsibilities include delivering and coordinating patient care, observing and reporting changes, documenting actions and assessments, updating nursing care plans, and educating patients and families on disease processes, self-care, and prevention. Requires active LPN/LVN license, CPR certification, TB testing, and ability to travel to home care locations. Flexible scheduling includes Days/Nights with full-time, part-time, or PRN shifts.
Required Qualifications
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice (LPN/LVN)
- Current CPR certification
- TB skin test (within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
Additional Requirements
- Travel required to patient homes in assigned areas
- State-specific requirements as listed (e.g., pediatrics experience in SC, etc.)
