Job Summary
Provide and document skilled nursing care in patients' homes under the supervision of a Registered Nurse, following individualized care plans and physician orders; perform nursing actions, monitor patient condition, update care plans, document activities promptly, and communicate changes to supervisors; educate patients and families on disease processes, self-care techniques, and prevention; adhere to infection control standards and professional boundaries; maintain licensure and CPR certifications, meet state requirements, and participate in ongoing education; travel to multiple home-care sites within Texas, with varied shifts (days/nights) and flexibility for full-time, part-time, or PRN schedules; successful candidates will have an active LPN/LVN license, CPR certification, and strong clinical and documentation skills.
Required Qualifications
- Active LPN/LVN license in state of practice
- Current CPR certification with hands-on component
- TB skin test within last 12 months
- Six months prior hands-on nursing experience preferred
- Reliable transportation
- Grad of an accredited nursing program
- Demonstrated nursing proficiency and compliance with policies
