Job Summary
Licensed Practical Nurse (LPN/LVN) providing skilled nursing care in private duty home health under the supervision of a Registered Nurse. Responsibilities include delivering and coordinating nursing care per physician orders, observing and reporting patient condition, documenting activities and care actions in a timely manner, participating in and updating the nursing care plan, ensuring infection control and safety, educating patients and families on disease processes and self-care, and maintaining professional competencies through continuing education.
Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- TB skin test (within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation
