Job Summary
Provide skilled nursing care, coordinate patient care plans, and document all activities and assessments in electronic systems while adhering to physician orders and confidentiality standards. Continuously observe patient conditions, report changes to supervisors, implement infection control measures per OSHA standards, and educate families on disease processes and self-care techniques. Recognize and respond to evolving patient needs to prevent or reduce risks, maintaining personal accountability through compliance with state regulations and Aveanna policies. Travel to patient homes in San Antonio and surrounding areas, performing duties that may involve lifting 50 pounds, prolonged standing, and emergency response in diverse environments.
Required Qualifications
- Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Must have reliable transportation
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
- Must comply with state and federal regulations, accrediting bodies and the Nurse Practice Act
- Must comply with OSHA standards
- Must comply with confidentiality standards and professional boundaries
- Must comply with Aveanna policies
- Must adhere to physicians orders
- Must adhere to developed care plan
- Must adhere to state specific requirements for South Carolina, California, or Louisiana (one year of experience)
- Must be a graduate of an accredited school of nursing
- Must have current, unrestricted state license as a Licensed Nurse in the state of practice
- Must have demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to sit and climb stairs
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills and skill evaluations
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
- Aveanna can assist in obtaining CPR certification after hire, if necessary
