Job Summary

Provide skilled nursing care and coordinate patient treatment plans in compliance with physician orders while documenting all assessments, actions, and care coordination in electronic systems. Continuously observe patient conditions, report changes to supervisors or physicians, and implement interventions to prevent or reduce risks. Educate patients and families on disease processes and self-care techniques while maintaining infection control measures and professional accountability. Schedule flexibility includes full-time, part-time, or PRN shifts across days, nights, and weekends. Requires an active RN license, CPR certification, and proficiency in clinical assessments.

Required Qualifications

  • Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
  • Compact licenses must be transferred to your state of residence within 90 days
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Must have reliable transportation
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • Current, unrestricted state license as a Registered Nurse in the state of practice
  • Must be able to speak, write, read and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to sufficiently reposition patients and move equipment without assistance
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate

Desired Qualifications

  • Six months prior hands-on nursing experience preferred but not required
  • South Carolina – One (1) year of pediatrics experience
  • California – One (1) year of experience required working under current nursing license
  • Louisiana – One (1) year of experience required working as a licensed nurse
  • Continuing Education as required by state
  • Medical Oncology RN - 2 years Registered Nurse experience with cardiac monitoring and oncology experience
  • Ambulatory RN - 2 years experience with vital assessment, plan of care development, and telephone triage
  • Medical Surgical RN - 2 years Registered Nurse experience
  • Peri-Operative Care RN - 2 years of Registered Nurse, preferably with oncology experience
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
  • Attention to detail
  • Time Management
  • Effective problem-solving and conflict resolution
  • Good organization and communication skills
  • Must be able to sit and climb stairs
  • Must have adequate sense of smell and touch
