Job Summary
Provide skilled nursing care and coordinate patient services in compliance with physician orders and care plans for individuals in home settings. Continuously assess patient conditions, document all activities and nursing actions, and implement interventions to prevent or reduce risks based on changing needs. Educate patients and families on disease processes and self-care techniques while maintaining infection control measures and adhering to state regulations. Participate in updating nursing care plans and engage in educational programs to maintain clinical competencies.
Required Qualifications
- Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Must have reliable transportation
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must have visual and hearing acuity
- Must have strong sense of smell and touch
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
- Must comply with state specific requirements (South Carolina: One (1) year of pediatrics experience; California: One (1) year of experience required working under current nursing license; Louisiana: One (1) year of experience required working as a licensed nurse)
- Continuing Education as required by state
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
