Job Summary
Provide and document skilled nursing care under RN supervision, adhering to physician orders and confidentiality standards. Continuously assess patient conditions, report changes to supervisors, and implement nursing actions to achieve expected outcomes while preventing risks. Coordinate quality patient care in compliance with OSHA infection control measures and state regulations. Educate patients and families on disease processes, self-care techniques, and prevention strategies. Maintain knowledge of nursing competencies through continued education and skill evaluations. This role supports Aveanna's national homecare mission, delivering compassionate care to children and adults across San Antonio and surrounding areas.
Required Qualifications
- Active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
- Compact licenses must be transferred to your state of residence within 90 days
- Current CPR certification (with hands-on component)
- TB skin test (current within last 12 months)
- Reliable transportation
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
- One (1) year of pediatrics experience (South Carolina)
- One (1) year of experience required working under current nursing license (California)
- One (1) year of experience required working as a licensed nurse (Louisiana)
- Continuing Education as required by state
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
- Attention to detail

- Good organization and communication skills
- Must be able to speak, write, read and understand English
- Must be able to travel
- Must be able to lift 50 pounds
- Must be able to sufficiently reposition patients and move equipment without assistance
- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
- Required vaccinations and testing, subject to exemptions for medical or religious reasons
Desired Qualifications
- Six months prior hands-on nursing experience preferred but not required
