Job Summary
Perinatal Navigator Float at Legacy Community Health coordinates prenatal care delivery and connects mothers and infants with essential services. Responsibilities include conducting point-of-care and digital health testing (including HIV and pregnancy testing), performing health screenings and risk assessments, implementing outreach strategies to engage pregnant individuals, providing culturally sensitive education on prenatal care, nutrition, and healthy pregnancy practices, conducting tech-enabled screenings of behavioral, social, and medical needs, motivating patients to set and achieve health goals, developing digital-first retention strategies for OB and pediatric patients, navigating care by linking to community resources (WIC, housing, transportation), documenting interactions in Legacy's EMR, collaborating with an interdisciplinary team, meeting performance metrics tied to grants and contracts, participating in clinics, meetings, trainings, and travel to different Legacy clinics, and embracing other duties as assigned.
Required Qualifications
- High School Diploma or equivalent
- 2-3 years of relevant experience in a healthcare setting
- Proficiency with computers and advanced health technology systems
- Ability to deliver point-of-care testing within 3 months
- Bachelor's Degree in a related field preferred
- Bilingual English/Spanish
- Experience with electronic health records (EHR)
- Experience with OB and Pediatric patient populations
- Experience working with sexual minorities and gender non-binary persons
- Keen ability to educate about pregnancy and postpartum using tech-based methodologies
- Travel to different Legacy clinics to promote cohesive, cross-location healthcare strategies
