Job Summary
Deliver comprehensive outpatient care to adult and senior patients while diagnosing and managing moderately to complex medical conditions. Maintain accurate medical records, coordinate referrals with specialists, and collaborate with interdisciplinary teams to ensure holistic care. Participate in clinical quality improvement projects and contribute to innovations in care delivery. Work on-site at our Winter Haven, FL clinic with lower daily patient volume to foster deeper relationships. Must be willing to participate in limited on-call coverage by phone.
Required Qualifications
- Three years of experience in value-based care or managing adult patients with chronic medical conditions
- Masters' degree as a nurse practitioner from an accredited medical school
- Active, autonomous nurse practitioner medical license in the state of practice, or to obtain
- Board Certification
- Must be willing to participate in limited on-call coverage by phone
