Job Summary
Provide clinical support services to individuals with autism; meet with a caseload on a scheduled basis (weekly, bi-weekly), including home visits and community locations; coordinate with DDS Autism Service Coordinators and Lifeworks Clinical Team to ensure a team approach; perform case management and connect individuals with services, benefits, and mental health resources; assess new referrals, establish person-centered goals, and coordinate with providers to deliver crisis intervention as needed; require strong interpersonal and written communication skills and the ability to work in a flexible schedule within a team-focused environment.
Required Qualifications
- Associates Degree plus 5 years of work experience in the Human Services field
- Valid Driver's License required
- fingerprint background check, CORI, DPPC and Driving background checks
