Job Summary

Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity Wrap Around model of care coordination. Develop and maintain the Wraparound Team, including coordinating and leading team meetings. Coordinate and supervise implementation of the Plan of Care, including Transition and Crisis Plans, while ensuring family support during crises. Provide and document Life Domain Assessments, maintain service documentation and data, and obtain weekly reports from subcontracted providers. Participate in after-hours on-call response, attend supervision and program meetings, and report all Multi-System issues immediately. This full-time role is based in Auglaize County, Ohio, with overlap into Miami County as needed, and includes a weekly on-call rotation.

Required Qualifications

  • Licensed or unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code
  • Completion of the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM
  • Successful completion of skill and competency-based training to provide MCC
  • Experience providing community-based services to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field
  • Three years of experience with a high school diploma or equivalent
  • Two years of experience with an associate's degree or bachelor's degree
  • One year of experience with a Master's degree or higher
  • Background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Cultural competence or responsiveness with training and experience necessary to manage complex cases
  • Qualifications and experience needed to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-serving systems (e.g., child welfare, juvenile justice, education)
  • Residence in one of the counties included in Catchment Area C
  • Valid driver's license
  • Reliable personal transportation
  • Good driving record
  • Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
