Job Summary
Act as the primary care provider for residents in Skilled Nursing Facilities (SNFs), overseeing medical management and direct patient care. Conduct patient rounds, diagnose and treat acute illnesses, manage ongoing conditions, and discuss treatment plans, prognosis, and advance care planning with residents and families. Maintain accurate medical records, ensure compliance with healthcare regulations, and collaborate with nurses, therapists, and specialists. Mentor and support NPs and PAs, with potential pathways into Medical Director/Attending leadership roles. This is a full-time role focused on delivering high-quality, compassionate care to seniors in a collaborative, mission-driven environment.
Required Qualifications
- Board-certified physician with experience in primary care and long-term care (LTC)
Additional Requirements
- Must be able to lift a minimum of 20 pounds and stand/walk/work on your feet up to 6-8 hours per day
