Job Summary
Provide primary-care services in long-term care facilities in accordance with site-specific protocols and standard of care for family medicine, geriatric medicine, or internal medicine. Review and sign MARs and facility orders, review lab results within 24 hours, manage facility-specific medication prescriptions, and allocate time for paperwork. Collaborate with facility staff; participate in care plan meetings and QA/PI activities; perform OnCall triage per policy and be available 7 AM–7 PM on scheduled days. Submit documentation and billing records per schedules; maintain open, HIPAA-compliant communication with staff and administrators; meet with facility care coordinators before and after daily patient care visits. Adhere to Eventus WholeHealth standards, regulatory requirements, and CPT coding guidelines. Requires Licensed MD or DO and at least 3 years in Long Term Care/Geriatric Care; AMDA certification is a plus. Strong leadership, organizational, and communications skills are essential.
