Job Summary
Provide primary-care services in long-term care facilities (skilled nursing and assisted living) within a physician-led team, aligning with site protocols across family medicine/geriatric/internal medicine. Duties include reviewing MARs and orders, lab reviews within 24 hours, managing scheduled medications per facility policy, allocating visit time for paperwork, collaborating with facility staff on care plans and QA/PI meetings, on-call triage per policy (7 AM–7 PM on scheduled days), submitting documentation and billing records, ensuring HIPAA-compliant communication, and maintaining knowledge of regulatory guidelines. Requires MD/DO license with 3 years in long-term care/geriatric care; AMDA certification is a plus; strong leadership, organizational, and communication skills; ability to adhere to CPT/CPT coding guidelines and EMR usage.
