Job Summary
Conduct initial assessments for new clients to establish care plans and perform routine check-ins to monitor client progress and provide guidance to in-home Personal Care Assistants. Complete visit reports electronically to ensure compliance and quality of care while managing your own calendar and coordinating visits based on personal availability. This flexible PRN role allows you to choose between 2 visits or 8, or take a week off entirely, with the option to work entirely in the field from home. Applicants must hold a current, unrestricted Georgia LPN or RN license with at least one year of experience, possess reliable transportation for travel within Atlanta and surrounding counties, and pass a background check and drug test.
Required Qualifications
- A current, unrestricted Georgia LPN or RN license
- At least one year of nursing experience
- Reliable transportation
