At Home Physical Therapist - Per Diem (Ocean County/Toms River)

Job Summary
Evaluate and treat patients recovering from total hip and knee replacement surgeries in their home setting, focusing on safe bed mobility, transfers, and ambulation. Administer tests, formulate diagnoses, and establish treatment plans while educating patients on community reintegration and ADLs. Collect and record patient care data in Prompt EMR, including daily notes, reassessments, and discharge plans, while completing daily billing and following department guidelines. Collaborate with the interdisciplinary team to modify care plans and ensure safe discharge to home or outpatient locations.
Required Qualifications
- Bachelor's or Master's Degree
- Doctorate degree preferred
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Excellent oral, written communication
- Strong clinical decision-making skills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
- Masters or better
- Bachelors or better
