Job Summary
Evaluate and treat patients recovering from hip and knee replacements in their homes, focusing on safe bed mobility, transfers, and stair negotiation. Collect and record care data for reimbursement while formulating diagnoses, goals, and treatment plans based on evaluation results. Administer tests, prescribe therapeutic activities, and educate patients on community reintegration. Collaborate with interdisciplinary teams to modify care plans and communicate progress reports. Complete daily billing, attend coding seminars, and schedule evaluations via the Patient Access team.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Excellent oral, written communication
- Strong clinical decision-making skills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
