Job Summary
Evaluate patients for orthopedic rehabilitation in the home setting, focusing on total hip and knee replacements. Conduct initial evaluations, administer tests, and synthesize results to formulate diagnoses, goals, and treatment plans. Educate patients on safe mobility, transfers, and ambulation while performing functional training for ADLs and IADLs. Transfer, ambulate, and negotiate stairs to assess discharge safety, then record care data in the Prompt EMR system. Collaborate with interdisciplinary teams and patient support systems to modify plans of care as needed. Complete daily billing, progress reports, and discharge planning to ensure continuity toward outpatient transition.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Orthopedic background
- Masters or better
- Bachelors or better
