Job Summary
Conduct orthopedic in-home physical therapy for patients recovering from total hip and knee replacements, focusing on safe bed mobility, transfers, ambulation, and stair negotiation. Evaluate patients to determine discharge readiness to home, synthesize assessment data to formulate diagnoses and treatment plans, and prescribe functional training for ADLs and IADLs. Administer therapeutic activities and rehabilitation equipment training while providing education on precautions and community reintegration. Record all care data in the Prompt EMR system, generate daily notes and progress reports, and manage billing compliance. Collaborate with the interdisciplinary team and patient support systems to modify care plans as needed.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Orthopedic background
- Masters or better
- Bachelors or better
