Job Summary
Evaluate and treat patients for total hip and knee replacement surgeries and other orthopedic conditions in their home setting. Educate patients on safe bed mobility, transfers, and ambulation while performing stair negotiation to determine discharge safety. Collect and record patient care data, initial evaluations, daily notes, and progress reports using Prompt EMR. Formulate diagnoses, goals, and plans of care based on assessment results, then prescribe therapeutic activities and rehabilitation equipment training. Collaborate with the interdisciplinary team to modify treatment plans and communicate with the Manager and Patient Access team for scheduling. Provide functional training for ADLs and IADLs to support community reintegration.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Orthopedic background
- Masters or better
- Bachelors or better
