Job Summary
Conduct one-on-one intake meetings with potential participants in their homes and guide them through the PACE enrollment process to meet enrollment targets. Assist in implementing strategies to enhance customer service throughout the organization. Travel throughout the tri-county area during day shifts, Monday through Friday, with driving as a necessary requirement. Provide proof of driver license and auto insurance upon request. This role supports the mission of keeping older adults healthy and independent in the community through clinics and home visits.
Required Qualifications
- Monday to Friday on day shift
- traveling throughout the tri county area
- Driving is a necessary as part of this role
- Caregivers are required to comply with all state laws and requirements for driving
- provide proof of driver license and auto insurance upon request
- 1 year experience working with a frail/elderly population or other direct service experience
Desired Qualifications
- Bachelor's Degree Social Work, Psychology, Health Care, Marketing, Business. Or equivalent educ/experience
- Experience working in intake, marketing, and/or with service systems for seniors
- Experience working in a PACE organization
