Job Summary
Lead clinical teams across multiple primary care centers while driving performance on key quality metrics and reducing hospitalizations within a fully capitated Medicare Advantage model. Coach and mentor physicians, support recruitment and onboarding, and maintain a small clinical panel of approximately 100 patients annually. Partner with operations leadership to ensure consistent execution of the care model, focusing on preventive care, chronic disease management, and patient engagement. This Monday-Friday role requires no nights, weekends, or call. Board-certified physicians with proven multi-site leadership experience in value-based care are ideal.
Required Qualifications
- Board-certified physician (Internal Medicine, Family Medicine, or related)
- Proven physician leadership experience in a multi-site or team-based environment
