Job Summary
Associate Care Coordinator responsibilities include processing referrals and documentation, supporting Territory Patient Care Coordinators and sales teams to drive territory growth, building relationships with physicians, hospitals, and referral sources, conducting routine visits to strengthen partnerships, obtaining re-authorization documentation for durable medical equipment, delivering educational materials, coordinating between clinical and sales teams, assisting with lead generation and account management, maintaining CRM records, and supporting territory coverage during PTO or training. Strong communication, relationship-building, business development, and organizational skills are essential; Bachelor's degree is preferred, with 1–3 years in healthcare-related roles and a valid driver’s license required to travel within the assigned territory.
Required Qualifications
- Bachelor's degree preferred
- 1–3 years of healthcare sales, clinical, customer service, or relationship-management experience preferred
- Experience working with physicians, hospitals, case management, pulmonary practices, or post-acute care settings preferred
- Valid driver's license and ability to travel throughout assigned territory
