Conduct humanized assistance to users and families, identifying situations interfering with therapeutic plans and coordinating with inter-sectorial teams. Execute active patient search via phone or teleconsultation, schedule transportation for out-of-municipality residents, and provide family and individual support at specialized clinics. Analyze social demands to grant social security benefits, manage patient discharge follow-ups, and ensure alignment with the Psychology sector. Maintain electronic systems, track budget indicators, and participate in quality management committees while training academic interns.
