Job Summary
Provide leadership and direction for the social services department, implementing policies to meet regulatory requirements and enhance departmental effectiveness. Conduct comprehensive assessments of residents' psychosocial needs, develop individualized care plans, and regularly review them to align with evolving resident needs. Offer emotional support and counseling to residents and families, while assisting with financial qualifications for government programs such as HCBS and Food Stamps. Collaborate with interdisciplinary teams including nurses, therapists, and physicians to address complex social issues, and participate in meetings to ensure a holistic approach to care. Coordinate services for OSSP and EPD waivers, maintain detailed records in PCC, and monitor waiver renewals to ensure timely compliance with DC regulations.
Required Qualifications
- Minimum of Associate or Bachelor's in Social Work
- Social Work license in DC
- Minimum of 5+ years of experience in a social service role within a long-term care or healthcare setting
- Experience working with older adults, in relevant capacity
- Experience with HCBS Medicaid Program
- Experience with fostering partnerships with Assisted Living resident agencies, such as, Social Security Administration, DACL, etc.
