Job Summary
LPN or RN floor nurse role at Horizon House with path to Resident Care Coordinator (RCC); responsibilities include administering medications and treatments, monitoring condition, documenting in EHR (PointClickCare), collaborating with care team, providing shift-based leadership to staff, ensuring safe and timely care, and growing into RCC with lead admissions, assessments, and family/provider communications; schedules include 7 days on, 7 off (Memory Care option) or alternative night/weekend shifts; opportunity for clinical leadership and career growth in a supportive, team-oriented environment.
Required Qualifications
- Active WA State LPN or RN license
- Strong medication administration and assessment skills
- Ability to guide and support care staff in a team environment
- Clear communication and confidence in addressing concerns in real time
- Comfortable with change and growth
- Assisted Living, LTC, or geriatric experience preferred
