Assisted Living Coordinator (LPN)

Coordinate and address health concerns for Assisted Living Residents, including resident assessments, medication management, and activities of daily living. Collaborate with the Director of Nursing and Assistant Director of Nursing regarding day-to-day clinical operations. Ensure pre-admission screens are completed prior to move-in and direct Lifestyle and Care Needs Assessments to determine medical and personal care requirements. Assist residents with bathing, dressing, grooming, dining, and medication administration while leading service plan development in conjunction with residents and families. Facilitate resident meetings, follow up on feedback, and serve as the lead nurse on the designated on-call rotation.
