Assisted Living Coordinator (LPN)

Coordinate and address health concerns for Assisted Living Residents, including resident assessments, medication management, and activities of daily living. Collaborate with the Director of Nursing and Assistant Director of Nursing regarding day-to-day clinical operations. Ensure pre-admission screens are completed prior to move-in and direct Lifestyle and Care Needs Assessments to determine medical and personal care requirements. Assist with bathing, dressing, grooming, dining, and medication administration. Lead and assist in developing service plans in conjunction with residents and families, ensuring communication to caregivers. Facilitate resident meetings and follow up on feedback. Serve as the lead nurse for assisted living residents and participate in the on-call rotation as designated by the DON.
