Job Summary
Lead and manage the overall direction of clinical resident care services by delivering a 1440 experience, optimizing performance, and leading employee connections within the community. Perform quarterly admissions and ongoing evaluations of residents, obtain written orders for care plan changes, and verify medication accuracy through pharmacy communication. Oversee resident and family care, communicate regularly with families, and conduct care conferences to ensure right care at the right price with the right staff. Schedule educational sessions for residents, review occurrence reports to address personnel issues, and lead the team toward continual improvement of the organization. Available to work a flexible schedule including on-call, weekends, and holidays when necessary.
Required Qualifications
- Licensed Practical Nurse (LPN) registered within the State preferred
- Prior supervisory experience
- Earned a degree from an educational institution in health care, mental health or social services profession or be a licensed Health Facility Administrator
- Minimum of 1 year of experience with dementia or Alzheimer's residents, or both, within the past 5 years
- Minimum of 12 hours of dementia-specific training within the last 3 months
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
- Ability to write routine reports and correspondence
- Ability to communicate effectively before groups of customers or employees of organization
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Proven organization and communication skills
- Ability to work successfully through complex situations (ability to problem solve)
- Available to work a flexible schedule including on call, weekends, and holidays when necessary
- This position requires the ability to review detailed documents and read computer screens
- The employee will occasionally lift and/or move up to 25 pounds
Desired Qualifications
- Assisted Living/Memory Care experience preferred
