Job Summary
Assist with direct care and support in a residential program for adults with ID/DD, promoting health, safety, and ISP-goal attainment. Provide hands-on care (showering, toileting, grooming, meal-time assistance), transport individuals as needed, and contribute to education, training, and counseling to support functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication, and community mobility. Develop and present ISP plans and assessments in collaboration with residential staff; maintain confidential files, participate in meetings, and coordinate with families, guardians, and funding sources. Prioritize safety and individualized programming while utilizing strong communication and documentation skills. Requires related experience, a high school diploma or GED, a valid driver’s license, and ability to pass required background checks; MAP certification is preferred.
