Job Summary
Coordinate in-house programming by conducting daily rounds, monitoring recreational activities, and ensuring safety and health requirements are observed. Manage individual care plans through assessments, goal planning, and monthly resident reviews while maintaining up-to-date records and incident reports. Direct transportation logistics for community inclusion, medical appointments, and home visits, including tracking appointments and managing medication inventory. Facilitate interdisciplinary team meetings, prepare agendas, and collaborate with consultants on programming recommendations. Prepare staff schedules, conduct performance evaluations, and ensure compliance with OPWDD and Justice Center guidelines.
Required Qualifications
- Highschool Diploma
- one (1) year experience in Human Services (Social Work, Psychology, Special Ed., etc.)
- Associate's Degree preferred
- 1 year administrative experience
- 2 years minimum experience working with individuals diagnosed with intellectual disability (ID)
- Has respect and consideration for the resident being served
- AMAP, SCIP-R and First Aid/CPR Certified
- A Valid Driver's License
- Must be able to drive large vehicle which consists of wheelchair or 15-seater van
- Must be able to travel
