Job Summary
Direct supervision of all program staff regarding case management and services for adults experiencing homelessness and mental illness. Audit and review individual consumer progress notes and service plans while ensuring plans reflect agency standards and trauma-informed care. Maintain contact with external agencies to secure client placements and coordinate intake scheduling and tenant orientation. Participate in crisis intervention, provide emergency coverage, and lead supervisory meetings with comprehensive documentation. Support Facilities Management to maintain apartment standards per S:US, OMH, and HUD guidelines. Represent the agency with community groups to develop service linkages.
Required Qualifications
- Bachelor's degree in Human Services or related field
- Minimum of two (2) years of supervisory experience
- Experience with homelessness and mental illness population
- Effective written and oral communication skills
Desired Qualifications
- Master's degree
- Professional license LMSW, LCSW, or LMHC
- Experience working with different populations
- Bilingual English/Spanish speaking
- Behavior Tech Training
- Basic computer skills (e.g. MS Office suite and Windows)
