Job Summary
Provide clinical supervision and coaching to case management staff while reviewing and approving documentation to ensure high standards of care. Conduct internal chart reviews, analyze dashboards and monthly reports to track outcomes, and develop supervision plans that manage reporting requirements and service standards. Identify community resources, facilitate referrals, and serve as a liaison with care coordination agencies, health providers, and employment services to secure necessary services for residents. Evaluate clients for suicidal or homicidal risk, support staff in developing safety plans, and respond to and de-escalate crises as needed. Develop and implement client-centered groups and programming that foster a supportive, inclusive community while collaborating with property management partners to help clients maintain housing and transition to independent living.
Required Qualifications
- MSW or MHC from an accredited school
- License required within one year of employment
- One year of supervisory experience in social services
- Knowledge of mental health, substance use, young adult and/or chronically homeless populations
- Experience working with OMH, DOHMH, and/or HASA contract managers
- Knowledge of funders' regulations, scopes of services, and standards
- Strong trauma-informed clinical engagement and interpersonal skills
- Ability to work effectively as part of a team
- Ability to make sound assessments of tenants' strengths and needs
- Commitment to working within a harm reduction model
