Job Summary

Assistant Director, Prison Health Reform and Programs leads a small team to deliver ACCHO-led transition care models for First Nations people exiting custody, coordinating and implementing reform programs across ACCHOs. The role requires strong stakeholder engagement, project management, budgeting and reporting, and the ability to manage competing priorities while maintaining culturally responsive, trauma-informed engagement with corrections and health partners to improve health outcomes for people exiting detention. Responsibilities include co-designing transition care models, overseeing program implementation, monitoring and evaluating outcomes, and establishing a national community of practice to strengthen capability across ACCHOs. The ideal candidate has qualifications in public health, project management, or related disciplines, with experience coordinating frameworks and delivering impact through stakeholder collaboration.
