Job Summary
Coordinate and oversee the Social Service department by supervising staff, conducting resident psychosocial assessments, and developing individualized plans of care. Direct interdisciplinary care planning conferences, facilitate discharge planning, and maintain comprehensive documentation in medical records. Serve as a liaison between residents, families, and external agencies while ensuring compliance with state, federal, and organizational policies. Manage admissions referrals, conduct facility tours for interested parties, and uphold safety protocols regarding fire and sanitation codes.
Required Qualifications
- Master's Degree in Social Work or a human services field
- Possess a CSW or LSW within the state of operations
- 3+ years of Social Work Experience
Desired Qualifications
- Long term care/skilled nursing experience preferred
