Job Summary
Coordinate quality patient care and manage the clinical team for the Desert Valley Hospice Interdisciplinary Group, including Registered Nurses, LPNs, CNAs, social workers, and chaplains. Interpret operational indicators to maintain per patient day costs within budget while ensuring strict compliance with Federal, State, and local regulations. Lead Interdisciplinary Group meetings, conduct home visits and record reviews for admissions, and oversee clinical documentation to ensure accuracy and completeness. Act as Administrator On-call and participate in the on-call rotation to respond promptly to after-hours patient needs and team inquiries. Supervise clinicians in establishing patient goals, developing Plans of Care, and managing care transitions to other providers.
Required Qualifications
- Registered Nurse of an accredited school of nursing
- current license to practice professional nursing in the state
- BSN preferred
- Two (2) years of clinical management or supervisory experience in hospice/home care setting preferred
- Demonstrates the ability to clearly document according to standards of practice
- accurately and efficiently audit the documentation of others
- Demonstrates knowledge of State and Federal regulations
- accrediting body standards
- Knowledge of ACHC or CHAP standards preferred
- Ability to learn and master the workflow in the EMR
- Ability to accurately perform workflow

- Comprehensive knowledge of the principals and practices of hospice care delivery
