Job Summary
Conduct initial assessments and develop individualized care plans for residents while delivering health promotion services and disease prevention counseling. Train, schedule, and guide the clinical team toward exceptional care experiences through regular engagement with physicians, healthcare providers, and family members regarding resident health status. Contribute to quality assurance and performance improvement initiatives while complying with confidentiality and privacy regulations. Maintain continuous learning through educational seminars and staff meetings. Requires valid RN or LPN licensure, one year of supervisory experience, and current CPR/AED certification. Prior senior care experience preferred. Full-time role with comprehensive benefits including medical, dental, vision, 401(k) match, and paid time off.
Required Qualifications
- Holds valid state licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
- Minimum of one (1) year supervisory experience
- Must hold a current CPR and AED certification
- Exceptional written and verbal communication skills
- Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity
Desired Qualifications
- Prior senior care experience strongly preferred
