Job Summary
Assistant Clinical Manager to support clinical operations at Select Home Health & Hospice Carmel, IN. Key duties include assisting with EMR workflows (Homecare Homebase), clinical documentation review, quality assurance and performance improvement (QAPI), regulatory compliance, data reporting, and coordination with the interdisciplinary team to maintain high-quality patient care. Responsibilities cover workflow monitoring, documentation support, process-improvement implementation, chart auditing, audit reporting, staff development and onboarding, and participation in compliance initiatives. Experience with clinical operations, chart audits, data management, and regulatory standards is required; credentials such as RN, LPN, SLP, PT, PTA, OT, or COTA are accepted.
Required Qualifications
- Minimum of one (1) year of home health clinical experience
- Credentials: Registered Nurse, Licensed Practical Nurse, Speech Language Pathologist, Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Certified Occupational Therapist Assistant
