Assessor Coordinator Degree

Assessor Coordinator role focused on coordinating and evaluating long-term care screenings and client supportive case management in Moose Jaw. Drives the use of the MDS electronic assessment system for continuing care eligibility and placement, coordinates client placement into vacancies, ensures services align with SHA policies and philosophy, and supports a multi-disciplinary team with quality-improvement practices. Requires a Bachelor’s degree in a related Health or Social Science, plus eligible registration with the appropriate licensing body if applicable; proficiency in Microsoft Office; strong communication, time management, negotiation, and interpersonal skills; ability to work independently and travel within the service area.
