Job Summary

Provide comprehensive assessment services to a caseload of homeless men referred to the Assessment Center by DHS's Intake Center. Primary duties include coordinating a comprehensive assessment and assisting with referrals to appropriate settings (e.g., return to family, long-term treatment facilities, supportive housing services, DHS program shelters). Responsibilities also include obtaining benefits and identification, engaging in day treatment programs, entering data into AWARDS and DHS Cares, generating ongoing progress and group notes, and monthly statistical reports. Participate in clinical team and case conferences, assist with program staff coverage, and provide emergency first aid/CPR when needed. Hours are 37.5 per week (Sun/Tues/Wed 10-6:30pm; Mon 12pm-8:30pm; Thurs 8am-4:30pm). Qualifications include HS Diploma/GED required with bachelor’s degree preferred and master’s ideal; bilingual Spanish or Polish preferred but not required; excellent communication and writing skills and computer proficiency; experience with residential treatment and homeless populations; CPR training certification or willingness to take CPR training; vaccination preferred but not required.
