Assessment Social Worker

Job Summary
Conduct comprehensive home assessments for new and ongoing older adult participants, identifying supportive services and program opportunities that align with their needs and abilities. Provide short-term case assistance, information, referral, and advocacy to clients and caregivers, while collaborating with the social work team to initiate program participation. Maintain accurate case records in Salesforce and facilitate multi-session support or psychoeducational groups. Manage a caseload of up to 20 Friendly Visiting matches through quarterly check-ins and ad-hoc support, while supervising interns and volunteers as needed. Represent DOROT at community outreach events and conferences. Work in-office three days per week with public transportation use for occasional visits.
Required Qualifications
- An LMSW or a Licensed Mental Health Counselor
- Affinity for working with an older adult population
- Previous professional social work experience
- Strong interpersonal, clinical, group facilitation, and presentation skills
- Strong written and verbal communication skills
- Strong organizational skills, with an ability to work independently and collaboratively
- Passion and sensitivity for DOROT's mission
- Knowledge of local aging and community resources
- Experience working with volunteers
- Availability to work approximately 8 Sundays and/or work nights per year for programs or events; this includes up to four events that may not be within your regular scope of work
- Proficiency with Microsoft Office Suite
- Experience with Salesforce or a similar constituent management system/database
