Job Summary
Collaborates in establishing the home health plan of care and coordinates care delivery across the continuum. Conducts comprehensive patient assessments, determines nursing diagnoses, and continually utilizes the nursing process to evaluate progress toward goals. Provides in-home nursing care, promotes health, ensures a safe environment, and delivers patient education. Role emphasizes evaluation of patient progress, applying interventions, and supporting health outcomes; 36 hours per week in Vanderbilt Home Care. Licensure as a Registered Nurse and minimum 1 year of relevant experience required; services extend to Cheatham, Davidson, Robertson, Rutherford, Sumner, Williamson, and Wilson counties.
Required Qualifications
- RN licensure (Licensed Registered Nurse)
- Graduate of an approved discipline-specific nursing program
- Minimum 1 year relevant work experience in home health or nursing
