Job Summary
Assess and plan Support at Home for older Australians through online meetings and phone conversations. Conduct personalised assessments, develop goal-oriented care plans with clients and their representatives, recommend tailored services to maintain independence, identify health and safety risks, build client budgets and service plans, and ensure alignment with allocated funding. Maintain accurate documentation in the care management system, work independently and with a multidisciplinary team, and support clients to achieve meaningful outcomes. This full-time role offers an in-office setting at Modern office in Bowen Hills, Bowen Hills, Queensland, with Monday-to-Friday hours and no shift work or weekends. Requires strong communication, digital confidence with Microsoft 365 and CRM tools, and a passion for quality aged-care support.
