Job Summary
Assessment Coordinator role supporting the Home Care Team: act as primary contact for clients of CHSP and other programs; respond to client queries; conduct comprehensive, person-centred assessments of client needs, suitability and eligibility for aged care services; triage by vulnerability and urgency; identify and manage risks to clients and staff during intake and reviews; complete scheduling and reviews for existing clients; maintain accurate records and reporting in line with CHSP funding and program guidelines; promote Mercy's Healthy Aging services to support clients across their aging journey.
