Job Summary
Conduct comprehensive initial and ongoing reassessments for new and existing home care patients while developing and updating individualized plans of care. Perform supervisory field visits to ensure services align with clinical standards, Joint Commission recommendations, and physician orders. Provide clinical education and competency validation for caregivers and nursing staff through coaching, mentoring, and skills check-offs. Assist with the admission process and serve as a trusted resource for patients, families, and field staff regarding care plans and safety. Participate in quality assurance initiatives, including clinical chart audits and agency readiness for regulatory surveys. Collaborate with the Director of Nursing and operations leadership to resolve issues and drive process improvements. Requires a current unrestricted RN license, two years of experience, and ability to travel to patient homes.
Required Qualifications
- Current unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of two years of nursing experience
- Previous experience performing patient assessments and developing care plans
- Valid driver's license
- Reliable transportation
- Ability to travel within the service area
- Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull with patient assessments as needed
- Must be able to lift up to 50 pounds
- Visual and hearing acuity required
Desired Qualifications
- Adult and/or pediatric home health, hospice, private duty, or community-based care experience
