Job Summary
Lead the Assertive Community Treatment (ACT) team by guiding multidisciplinary professionals to support individuals with complex behavioral health needs. Facilitate relationships between the agency, families, local agencies, and the community while minimizing psychiatric symptom interference through supportive counseling and skill-building activities. Perform case management functions, including arranging referrals, ensuring timely service reauthorizations, and coordinating care transitions. Maintain accurate clinical documentation, monitor service utilization against rules and authorizations, and collaborate with staff to standardize procedures. Participate in a first responder on-call system available 24/7/365 to coordinate resources for consumers and natural support networks. Provide leadership through coaching, performance management, and hiring duties while maintaining compliance with clinical and regulatory standards.
Required Qualifications
- A master's degree from an accredited university in Counseling or a related field
- A minimum of two years of experience working with individuals with mental and/or behavioral health issues that include writing person-centered plans, conducting effective group therapy and providing mental health diagnoses
- Valid licensure to provide mental/behavioral healthcare in the state of North Carolina; Licenses include LCSW, LCSWA, LCMHC, LCMHCA, LMFT or Licensed Psychologist
- General working knowledge of the human services delivery system in North Carolina
- Valid driver's license, auto insurance and reliable transportation
- Must be authorized to work in the United States
- Must complete background checks and credential verification
